On October 5, 2015, the numbering of the pages by Adobe Acrobat was changed so that an Arabic page number typed into the appropriate control field in Adobe Acrobat or Acrobat Reader will bring up the correspondingly numbered page of the text. At the same time, a typing error was fixed on page 76 (in Chapter 11).
On November 29, 2011, a typing error was fixed on page 271 (in Chapter 19).
On February 10, 2011, a typing error was fixed on page 57 (in Chapter 9).
On October 12, 2010, a typing error was fixed on page 63 (in Chapter 10).
On January 29, 2010, a typing error was fixed on page 105 of the text (in Chapter 12).
On October 9, 2009, typing errors were fixed on pages 182 (in Chapter 14) and 198 (in Chapter 15).
On October 6, 2009, typing errors were fixed on pages 106, 120 and 144 (in Chapter 12); 203 (in Chapter 15); and 232 (in chapter 17).
On October 3, 2009, typing errors were fixed on pages 35 (in Chapter 6), 82 (in Chapter 11), and 113 (in Chapter 12).
On October 16, 2008, typing errors were fixed on pages 15 (in Chapter 3); 36 (in Chapter 6); 43 (in Chapter 7); 63 and 66 (in Chapter 10); 84 and 90 (in Chapter 11); 96, 147 and 162 (in Chapter 12); 167, 172 and 174 (in Chapter 13); 178 (in Chapter 14); 191 and 197 (in Chapter 15); 226 (in Chapter 17); and 252 (in Chapter 18).
On July 8, 2008, a typing error was fixed on page 141 (in Chapter 12).
On July 8, 2006, a spelling error was fixed (with the approval of the author) on page 257 (in Chapter 19).
On August 22, 2005, typing errors were fixed on pages 24 (in Chapter 4), 38 (in Chapter 6), 53 (in Chapter 8), 109 and 161 (in Chapter 12), 206 (in Chapter 16), 226 and 235 (in Chapter 17), and 258 and 272 (in chapter 19).
On June 23, 2004, a typing error was fixed on page 259 (in Chapter 19).
